Acute coronary syndrome (ACS) was one of the most frightening cardiovascular disease. It was currently one of the leading causes of death in developed and developing countries, including Indonesia. ACS can be both acute myocardial infarction, including ST-segment elevation MI (STEMI) and non-segment elevation MI (NSTEMI), and unstable angina. Globally, it’s become the first major cause of death in developing countries, superseding g the mortality rates due to infection. Systematic effort was needed intensively to prevent the increasing cases of morbidity which could be managed with the cardiac rehabilitation program. One of the cardiac rehabilitation programs which could be established was directed physical exercise. The aim of this study was To determine the influence of directed physical exercise on the function of cardiac muscle which assessed based on blood pressure, pulses and ECG results. The study uses a quasi-experiment design, 64 people were become the research subject divided into 32 peoples as intervention group and 32 peoples as control group, carried out in cardiology clinic of Siti Khodijah hospital, Surabaya. Wilcoxon test and Mann Whitney test, obtained results that there was a significant influence on blood pressure with p- value of 0.001 (p <0.05), there was no significant influence on the pulse change with the p-value of 1.000> (p <0.05), and significantly influence the ECG changes with P-value 0.000 <(P <0.05). The results of the Nagelkerke test and Chi-square, showed that directed physical exercise has contribution to the blood pressure of 16.4%, OR = 9.552, while the ECG changes of 47.0%, OR = 27.617. In sum, Directed physical exercise has a significant influence on blood pressure and ECG, where the directed physical exercise has a more significant effect on blood pressure than on the ECG.

Key word : Directed physical exercise and Coronary acute syndrome (ACS)
